The American Bar Association's "Tool Kit for Health Care Advance Planning" (this set of documents is a very good starting point for preparing instructions and values statements for clients). WARNING: If you download the Tool Kit documents, we INSIST that you also download the ABA's order form and send them $24 plus $5 shipping and handling. If we don't hear that they are getting a sufficient number of forms sent in, these documents will be taken off our site and the only way anyone will be able to get them is to prepay the ABA.
The ABA Tool Kit is a set of ten documents, formatted in Microsoft Word. When opened in WordPerfect, they should retain formatting fairly well. If you get a message that you do not have Word installed, you can save the file to your local machine and open it in WordPerfect (but perhaps not in version 6.1) later. There is even a cool logo; you can replace it with your firm's logo for consistency of message.
